Hair Loss in Asian Men: Presentation, Prevalence, and What's Different
AGA rates, patterns, and the hair characteristics that change how thinning looks and how surgery should be planned — for men of East and Southeast Asian descent.
AGA affects men across all ethnic backgrounds, but the typical progression, presentation, and response to treatment varies enough between populations to be worth addressing specifically. For men of East and Southeast Asian descent, a few differences matter in practice.
01Prevalence and progression patterns
Androgenetic alopecia rates in East Asian populations have historically been reported as lower than in Western European populations — roughly 25–30% of Korean and Chinese men show AGA by their 50s, compared to closer to 50% in men of Northern European descent. Onset tends to be somewhat later. However, rates appear to be rising in younger generations in urbanized Asian populations, with diet, stress, and lifestyle factors likely contributing alongside genetics.
Asian hair tends to be straighter, thicker in diameter, and has a longer anagen phase than many other hair types. Thicker shaft diameter means better visual coverage per follicle — an advantage in density appearance even as follicle count decreases. But it also means the visual change when thick hair miniaturizes to thin hair can be more dramatic in contrast.
02Norwood staging and Asian hair
The Norwood scale was developed on predominantly white European patients. Crown and vertex thinning (Norwood III Vertex, IV) tends to be more common in East Asian presentations relative to frontal temporal recession. The "Type A" variant (frontal band recession without crown loss) is less common. This distribution difference matters for surgical planning — crown-heavy presentations need a different graft allocation than predominantly frontal recession.
03Treatment: same drugs, similar response
Finasteride and minoxidil are effective in Asian men with AGA. Several large finasteride trials specifically in Japanese and Korean populations showed response rates and efficacy profiles comparable to Western trials. Dutasteride is actually FDA-approved for hair loss in Japan — the Japanese regulatory pathway moved ahead of the US on this. The standard treatment stack applies without meaningful adjustment.

Asian hair's straight, thick characteristics actually work well for FUE extraction — the straight follicle angle simplifies extraction compared to curly hair.
