Biotin (vitamin B7) is among the top-selling supplements globally, and hair loss is its primary marketing claim. The evidence for biotin supplementation in men without documented deficiency is, by any rigorous standard, negligible. Here is why it remains the most-sold hair vitamin despite having the least supporting science.
What Biotin Actually Does
Biotin is a water-soluble B vitamin that functions as a cofactor for five carboxylase enzymes involved in fatty acid synthesis, amino acid metabolism, and gluconeogenesis. It contributes to protein synthesis broadly, which is the kernel of truth at the center of the marketing narrative — keratin is a protein, biotin helps produce proteins, therefore biotin helps hair. This logic has significant gaps.
Severe biotin deficiency — a genuinely rare condition — does cause hair loss, along with brittle nails, dermatitis, and neurological symptoms. The deficiency states studied are primarily inherited enzyme defects (biotinidase deficiency) and cases in people consuming large quantities of raw egg whites (avidin in raw eggs binds biotin, preventing absorption). These are not conditions affecting typical men purchasing supplements at a pharmacy.
The Evidence Gap
A 2017 systematic review published in Skin Appendage Disorders — the most-cited review on biotin for hair and nail conditions — identified 18 reported cases where biotin supplementation was associated with hair or nail improvement. All 18 had documented underlying pathology causing biotin deficiency or altered biotin metabolism. Not one study demonstrated benefit in biotin-sufficient individuals with hair loss.
Why It Keeps Selling
Several converging factors sustain the biotin market despite the evidence gap:
- Safe at any reasonable dose: As a water-soluble vitamin, biotin has no established tolerable upper intake level. Excess is excreted in urine. There is no meaningful risk from taking it, which removes a barrier to trying it.
- Regulatory gap: Dietary supplements are not required to demonstrate efficacy before sale in the United States. "Supports healthy hair" is a structure/function claim that requires no clinical substantiation.
- Anecdote and selection bias: People who experience spontaneous hair improvement while taking biotin attribute it to the supplement. People for whom it does nothing stop taking it without reporting the non-result.
- Nail association bleed-over: Biotin has somewhat better (though still weak) evidence for brittle nails. This associations bleeds into hair marketing.
Biotin interferes with several common laboratory assays — particularly thyroid function tests and troponin assays — because many immunoassays use biotin-streptavidin chemistry. High-dose biotin supplementation can produce falsely abnormal results. Men taking biotin should disclose it to any clinician ordering these labs.
Biotin Deficiency: When It's Real
True biotin deficiency in adults is uncommon but not impossible. Risk factors include: long-term anticonvulsant use (particularly valproate), chronic alcoholism, raw egg white consumption, pregnancy, and rare enzyme defects. If any of these apply, serum biotinidase activity can be tested, though it's not a standard panel item.
What To Do Instead
Men spending money on biotin for hair loss would see meaningfully better outcomes directing that budget toward proven interventions — finasteride has decades of RCT data, topical minoxidil has been FDA-cleared since 1988. The monthly cost of telehealth-prescribed finasteride is often less than premium biotin supplement subscriptions.
